Immunization Affidavit
According to Child Care Licensing, all children are required to be fully immunized to attend a child care center. In the event that a child is not fully immunized, and/or receiving delayed/staggered immunizations, the Texas Department of Health provides exemptions for the following circumstances:
To allow your child’s attendance in a licensed child care center, you must submit an affidavit for immunization from the Texas Department of Health to the RECC office. The form must be notarized and will be good for two years. ***Remember, a notary’s purpose is to witness a person signature. Please do not sign the form in advance.***

Exemptions to Immunization Requirements:
Chapter §97.62 of the Texas Administrative Code (TAC) describes the conditions under which individuals can seek exemptions from Texas immunization requirements. Exclusions from compliance are allowable on an individual basis for medical contraindications, reasons of conscience, including a religious belief, and active duty with the armed forces of the United States.
